| Monthly Availability Form |

This form should be completed when there are days in the month when you cannot skate. The
form assumes you are AVAILABLE unless you check the
UNAVAILABLE button for the specific date. Games & rinks will be assigned based
on the annual availability sheet that you submitted prior to the season.
In order to make sure the scheduler has the information in time, it should be completed by
the 15th of the prior month. For example, if you cannot skate certain days in October,
submit this form by September 15th.
